Endoscopic laser cordectomy, also known as the Kashima operation, is an endoscopic laser surgical procedure performed to treat the respiratory difficulty caused by bilateral abductor vocal cord paralysis. Bilateral vocal fold paralysis most often results from abnormal nerve input to the laryngeal muscles, weakening or eliminating their movement, most commonly through damage to the vagus nerve or its distal branch, the recurrent laryngeal nerve, though paralysis of the vocal fold can also result from mechanical breakdown of the cricoarytenoid joint. The procedure was first described by Kashima in 1989.

FAQs
When should endoscopic laser cordectomy be avoided?
Avoid it with a tumour spread through the thyroid cartilage or when general anesthesia or positioning is unsafe.
The Kashima operation should be avoided when a tumour is diffused throughout the thyroid cartilage, because operating could damage the tumour and may lead to its metastasis. It should also be avoided in patients with a history of bradycardia, aneurysms or recent infarcts, where general anesthesia may threaten the patient's life. The laser surgery cannot be performed on a patient with a fractured cervical spine, or with severe ankylosing spondylitis in which the spine is fused and rigid, because proper positioning of the patient would not be possible.
