Trabeculectomy

Surgical Procedure

Trabeculectomy is a surgical procedure used in the treatment of glaucoma to relieve intraocular pressure by removing part of the eye's trabecular meshwork and adjacent structures. It is the most common glaucoma surgery performed, and allows drainage of aqueous humor from within the eye to underneath the conjunctiva, where it is absorbed. This outpatient procedure has most commonly been performed under monitored anesthesia care using a retrobulbar or peribulbar block, or a combination of topical and subtenon anesthesia, with general anesthesia used only rarely.

FAQs

Is trabeculectomy usually the first treatment for glaucoma?

Usually not; medication is normally tried first.

Usually not. The EyeWiki reference notes that most physicians try medical therapy before proceeding to surgery. Trabeculectomy is considered when intraocular pressure is not adequately controlled on medications, or when there are significant barriers to using medications regularly, such as cost, compliance, ocular or systemic side effects, inconvenience, physical impairment or other psychosocial issues. This describes how the procedure is generally positioned, not advice for any individual, and the decision belongs with an eye surgeon.
