Iliocostal friction syndrome

Iliocostal friction syndrome, also called costoiliac impingement syndrome, occurs when the lower ribs come into contact with the iliac crest of the pelvis, and it presents as low back pain that may radiate to nearby areas because of irritated nerve, tendon and muscle structures. It can occur on one side of the body due to scoliosis, or on both sides due to osteoporosis and excessive kyphosis, and diagnosis is mostly clinical, based on assessing the underlying cause of the contact, though imaging can support the evaluation. Treatment focuses on the underlying cause and may include bracing, injection therapy, and surgery when more conservative approaches are not enough.
