Metoidioplasty is a female-to-male gender-affirming surgery that reconstructs a functional, penis-like structure from a clitoris enlarged by prior testosterone therapy. Testosterone replacement gradually enlarges the clitoris, which is developmentally homologous to the penis, to a mean maximum size of about 4.6 centimeters, and the surgery then reshapes this tissue. Compared with phalloplasty, metoidioplasty is technically simpler, less costly, and carries fewer potential complications, though it produces a smaller result with more limited capacity for penetrative intercourse.
