Molar distalization is an orthodontic procedure that moves molar teeth, especially the permanent first molars, backward within the dental arch. It is used to treat Class II malocclusion, a bite problem often caused by the early loss of primary molars that lets the permanent molars drift forward, and it can also create space for teeth such as impacted premolars or canines to come in properly. Distalizing molars in the upper (maxillary) arch is generally easier than in the lower (mandibular) arch, because the maxillary bone has proportionally more of the spongier trabecular bone, while the mandible is largely denser cortical bone. Popular appliances for the procedure include the Pendulum and Pendex devices, both developed by the orthodontist Hilgers in 1990.

Does it matter whether the second molars have come in before molars are moved backward?
Moving first molars before the second molars erupt is recommended, but a 2013 systematic review found the effect of eruption stage appeared to be minimal.
It can matter, though the evidence is mixed. The literature reports that the eruption stage of the second molar affects how the first permanent molar moves. When the second molar has not yet erupted, the first molar tends to tip backward, and treatment takes longer if the second molar has already erupted, so distalizing the first molars before the second molars erupt is recommended. A 2013 systematic review by Flores-Mir and colleagues found that the effect of second and third molar eruption stage on distalization appeared to be minimal. Of the four studies it examined, only one favored more movement with unerupted second molars, reporting 3 mm of distal movement against 2 mm and 5.2 months of treatment against 6 months, and the review noted limitations such as different appliances and different landmarks across the papers.
