An acute, highly contagious viral disease caused by Variola virus, spread person to person through respiratory droplets and contact with skin lesions, historically fatal in roughly three in ten cases and disfiguring or blinding many survivors. Smallpox devastated populations across recorded history and contributed to the catastrophic decline of Indigenous American populations following European contact. It became the first human disease deliberately eradicated: following a World Health Organization-led global vaccination campaign, the last naturally occurring case was recorded in Somalia in 1977, and the WHO certified worldwide eradication in 1980.

On October 26, 1977, a hospital cook named Ali Maow Maalin fell ill in the Somali port town of Merca. He had a rash. A local health officer, trained just weeks earlier to recognize exactly this, diagnosed smallpox and had him isolated within days. Maalin recovered. No one else in Merca ever caught the disease from him. He turned out to be the last person on Earth to acquire smallpox naturally, the final case in a chain of transmission that stretches back, by the evidence of Egyptian mummies, at least three thousand years.

That the story has an ending at all is the strange part. Smallpox killed roughly three people in ten it infected, scarred or blinded most of the rest, and had done this to every human population it reached for as long as anyone had kept records. It was not a disease anyone expected to end. Diseases do not usually end. They retreat, they mutate, they wait. Smallpox is different, and it is different for a specific, traceable reason: it has no animal reservoir. The virus lives only in humans. Find every last human case, ring it with vaccinated contacts so the chain of transmission has nowhere to go, and, in principle, the virus runs out of hosts and stops existing outside a laboratory freezer.

In principle is doing a great deal of work in that sentence, and the World Health Organization's Intensified Eradication Programme, launched in 1967, is the story of turning a principle into a fact on the ground in dozens of countries at once, several of them at war, most of them desperately poor, none of them possessing anything like a complete public health infrastructure. The tool the campaign actually relied on was not, in the end, mass vaccination of everyone, which had been the first approach and had proven too slow and too resource-hungry to outrun active outbreaks. It was surveillance and containment: find outbreaks fast, ring-vaccinate everyone the case could plausibly have contacted, and let the ring, not a blanket campaign, do the work of extinguishing the chain. This was, by the standards of 1967 public health doctrine, a genuinely new idea, and it worked faster than the older approach had.

The campaign is also, less comfortably, a Cold War story. The Soviet Union proposed global eradication to the World Health Assembly in 1958 and donated the bulk of the vaccine used in the campaign's early years; the United States became the programme's largest funder after 1965. Two governments that agreed on almost nothing else agreed that this disease should not exist, and kept funding the effort through the decade's worst diplomatic freezes. It is one of the few genuine full-throated collaborations the Cold War produced, and it is not an accident that it happened over smallpox specifically: a disease with no animal reservoir is a problem international cooperation can actually finish, in a way a problem like malaria, endlessly reseeded from mosquitoes and animal hosts, cannot be.

The World Health Organization certified the world free of smallpox in 1980, three years after Ali Maow Maalin recovered in Merca. It remains, more than four decades on, the only human disease eradication achieved through deliberate intervention rather than through the disease simply fading on its own (as, for instance, the plague's virulence did, gradually, without anyone doing anything about it). Every other disease on the founding roster of this atlas's own condition roster, tuberculosis, malaria, cholera, influenza, is still out there today. Smallpox is not. That gap between the two lists is the entire argument for why the campaign matters: not that disease can sometimes be beaten, which every recovered patient already knew, but that a disease can be beaten everywhere, permanently, on purpose.
