Transsphenoidal surgery is a technique for operating on structures deep in the head by passing an endoscope or surgical instruments through the nose and through the sphenoid bone, the butterfly-shaped bone that forms part of the base of the skull, into the sphenoidal sinus cavity behind it. It is used chiefly to remove tumors of the pituitary gland, which sits just beyond the sphenoid bone. Although the pituitary lies within the skull, it is not part of the brain itself, so this route lets surgeons reach it without disturbing the brain.
